- Order 5 bottles of Calorad®, either by phone at
1-800-860-0023 or by mail (see the order form and instructions below).
If you call in your order we will ship it immediately, but you will not
be registered as a distributor until after we receive your signed form.
Please let us know at the time of your phone order that you intend to
sign up.
- Fill in the Associate Information Section of the "INDEPENDENT BUSINESS
ASSOCIATE APPLICATION AND AGREEMENT", including your Social Security or Tax I.D.
Number. This is the top section. The "INDEPENDENT BUSINESS ASSOCIATE APPLICATION
AND AGREEMENT" Form is on the next page.
We will complete the Sponsoring Associate Information Section and the Placement
Information Section when we receive your form.
