HOW TO FILL IN THE PRODUCT ORDER FORM

In the "Associate Information" Section:

  1. Write the person's name and Social Security Number that you wish to credit the order to.


    2. In the "Shipping Information" Section:

  2. Fill in where you want the order shipped to.


    3. In the "Product Order" Section:

  3. Indicate which Business Center (usually 001, 002, or 003) the order should be credited to. This is the Business Center of the person listed in the "Associate Information" section, not necessarily of the person the order is being shipped to.


  4. Indicate if this is a requalification order.


  5. Fill in the Item # (CALORAD ® is CM301), the quantity (number of bottles), Product (CALORAD ®), the Associate Unit Cost (CALORAD ® is $40.00) and the Associate Cost Subtotal ($40.00 times the number of bottles ordered), The Associate Cost Total (the same as the Associate Cost Subtotal unless you ordered other EYI products).


  6. Shipping is 5% of the Associate Cost Total for UPS Ground, 8% for FedEx 2nd Day Air, and 15% for Next Day Air.

    There is a $2.50 Handling charge for each order.

  7. Indicate your method of payment. If by credit card, be sure to write in your account number and expiration date. Sign and date your form.

SHIPPING INFORMATION

Shipping costs are: 5%     ($2.00 per bottle) for UPS Ground
                             8%     ($3.20 per bottle) for 2nd Day FedEx and
                             15%   ($6.00 per bottle) for Next Day Air

CALORAD ® sent by UPS Ground should arrive in about 7 days after ordering. FedEx Second Day Air reduces delivery time by about 5 days after ordering. Next Day Air should arrive in about 4 days after ordering.
